USCIS Now Requires Report of Immigration Medical Examination and Vaccination Record to be Submitted with Form I-485 for Certain Applicants

We now require certain applicants filing Form I-485, Application to Register Permanent Residence or Adjust Status, to submit Form I-693, Report of Immigration Medical Examination and Vaccination Record, with their Form I-485 or the Form I-485 may otherwise be rejected.

Applicants for adjustment of status generally must complete an immigration medical examination and all required vaccinations and submit a properly completed Form I-693 signed by a civil surgeon to show they are free from health conditions that would render them inadmissible under the health-related grounds.

If you are required to submit Form I-693, or a partial Form I-693 (such as the Vaccination Record), you must submit it with your Form I-485. Otherwise, we may reject your Form I-485. We have revised the Form I-485 instructions to make filing the two forms together a requirement.

We now require certain applicants filing Form I-485, Application to Register Permanent Residence or Adjust Status, to submit Form I-693, Report of Immigration Medical Examination and Vaccination Record, with their Form I-485 or the Form I-485 may otherwise be rejected.

ادارۀ ملی احصائیه و معلومات، پروسۀ شارت لست متقاضیان 300 بست تذکرۀ الکترونیکی را انجام داده است. این اداره، از میان 16هزار و 3صد و 21 داوطلب به‌تعداد 14هزار و 7صد و 23 تن را با در نظرداشت شرایط بست، واجد شرایط تثبیت نموده است.

تمام متقاضیان شارت لست شده به تفکیک ذیل:
⦁ اناث (عمومی) و متقاضیان ذکور ‌که حرف اول نام‌های شان به «الف» آغاز می‌گردد، به‌روز سه شنبه، 13 عقرب 1399، قبل از ظهر؛
⦁ متقاضیان ذکور ‌که حرف اول نام‌های شان به «ب، پ، ت، ث، ج، چ، ح، ن، و، هـ، ی» آغاز می‌گردد، به‌روز سه شنبه، 13 عقرب 1399، بعد از ظهر؛
⦁ متقاضیان ذکور ‌که حرف اول نام‌های شان به «س، ش، ص، ض، ط، ظ، خ، د، ذ، ر، ز، ژ» آغاز می‌گردد، به‌روز چهارشنبه، 14 عقرب 1399، قبل از ظهر؛

به‌منظور اخذ کارت شمولیت به ادارۀ ملی احصائیه و معلومات، واقع ناحیۀ هفتم، نزدیک باغ بابر و سپس به منظور ثبت معلومات بایومتریک به دفتر سره میاشت، واقع ناحیۀ پنجم، تعمیر سابقۀ معینیت تعلیمات اسلامی وزارت معارف مراجعه نمایند.

امتحان تمام واجدین شرایط به روز جمعه، 16 عقرب 1399 در محل که درج کارت شمولیت می‌باشد، اخذ می‌گردد.

یادداشت: در صورتی که متقاضی کارت شمولیت اخذ ننماید و یاهم معلومات بایومتریک آن ثبت نشود، شامل امتحان نمی‌گردد.
هم‌چنان، کاندیدان فوق در روزهای ثبت معلومات بایومتریک و امتحان، اصل تذکرۀ تابعیت و یا پاسپورت خویش را با خود داشته باشند.
