Quy trình sản xuất que thử phát hiện nhanh virut rota

Que thử theo giải pháp này hoạt động trên nguyên tắc sắc ký miễn dịch chứa hai kháng thể đa dòng kháng virut rota được sản xuất trên hai động vật khác nhau.

Giải pháp hữu ích của nhóm các nhà khoa học thuộc Trường Đại học Bách Khoa Hà Nội có thể giúp sản xuất que thử phát hiện nhanh virut rota – nguyên nhân gây tử vong do tiêu chảy lớn nhất ở trẻ em dưới 5 tuổi.

Theo thống kê, hằng năm có khoảng 25 triệu người mắc virut rota, trong đó có 2 triệu người phải nhập viện. Trong số 600.000 trường hợp trẻ tử vong do virut rota, có hơn 80% đến từ các nước đang phát triển. Từ năm 1999, tại Châu Á, hệ thống giám sát bệnh tiêu chảy do virut rota đã được thiết lập, trong đó có sự tham gia của Việt Nam. Kết quả cho thấy tỷ lệ tiêu chảy do virut rota chiếm 55-60% trường hợp. Theo ước tính của Tổ chức Y tế Thế giới, virut rota chiếm 4-8% số ca tử vong của trẻ em dưới 5 tuổi. Tiêu chảy do virut rota ở trẻ em thực sự là bài toán khó đối với tất cả các nước trên thế giới. Ngay cả việc cung cấp nước sạch và cải thiện điều kiện vệ sinh cũng không giúp giảm thiểu số trường hợp bị tiêu chảy do virut này.

Bên cạnh việc giám sát dịch tễ nhằm tạo ra một loại vaccine hiệu quả thì việc chẩn đoán bệnh nhân tiêu chảy do virut rota gây ra là vô cùng cần thiết. Một khi được chẩn đoán chính xác, các bác sĩ mới có thể áp dụng phương pháp điều trị thích hợp và hiệu quả nhất.

Hiện nay trên thế giới đã xuất hiện nhiều phương pháp phát hiện sự có mặt của virut rota: kính hiển vi điện tử, phát hiện kháng nguyên (miễn dịch enzyme, ngưng kết latex và sắc ký miễn dịch), phát hiện axit nucleic (điện di polyacrylamit – PAGE, khuếch đại axit nucleic RT-PCR, giải trình tự gene). Phần lớn các phương pháp này đều được WHO khuyến cáo sử dụng trong những trường hợp nhất định, trong đó 2 phương pháp được khuyến cáo sử dụng trong xét nghiệm tại các cơ sở y tế đó là ELISA và sắc ký miễn dịch (que thử). Tuy nhiên, các phương pháp này lại tương đối khó, mất nhiều thời gian, đòi hỏi các thiết bị chuyên dụng và các nhân viên phải được đào tạo chuyên sâu, nguyên vật liệu tương đối đắt, que thử hoàn toàn phải nhập ngoại. Các trung tâm y tế rơi vào tình trạng không chủ động được nguồn sinh phẩm, đặc biệt là thiết bị nhập ngoại có thể không phù hợp với chủng virut rota hiện đang lưu hành tại Việt Nam.

Với mong muốn tạo ra que thử phát hiện nhanh virut rota nhằm khắc phục các nhược điểm trên, PGS.TS Trương Quốc Phong và các nhà khoa học thuộc Trung tâm Nghiên cứu và Phát triển Công nghệ Sinh học, Viện Công nghệ Sinh học và Công nghệ Thực phẩm (Trường Đại học Bách Khoa Hà Nội) đã đề xuất một quy trình sản xuất que thử phát hiện nhanh virut rota có giá thành thấp, độ nhạy và độ đặc hiệu cao, phù hợp với việc chẩn đoán các chủng virut rota lưu hành tại Việt Nam.

Que thử theo giải pháp này hoạt động trên nguyên tắc sắc ký miễn dịch chứa hai lang thể đa dòng kháng virut rota được sản xuất trên hai động vật khác nhau là thỏ và chuột lang, cùng một kháng thể đa dòng dê kháng IgG thỏ. Quy trình này bao gồm các bước: i) chuẩn bị mẫu huyết thanh từ thỏ và chuột lang đã được gây miễn dịch bởi virut rota toàn phần; ii) tinh sạch kháng thể đa dòng thỏ và chuột lang từ mẫu huyết thanh thu được ở bước (i) để thu được dung dịch chứa kháng thể đã tinh sạch; iii) cô đặc dung dịch chứa kháng thể đã được tinh sạch thu được ở bước (ii); iv) điều chế dung dịch nano vàng; v) tạo dung dịch cộng hợp kháng thể đa dòng thỏ kháng virut rota và hạt nano vàng; vi) tạo miếng thấm cộng hợp bằng cách ngâm miếng thấm trong dung dịch cộng hợp (v) và sấy khô; vii) cố định kháng thể lên màng nitroxenluloza; và viii) tạo que thử phát hiện nhanh virut rota.

Để sử dụng, người dùng sẽ đặt que thử nằm ngang, nhỏ 0,1 ml mẫu (3-5 giọt) vào vị trí nạp mẫu được đánh dấu trên que thử, quan sát và đọc kết quả sau 3-5 phút. Khi xuất hiện cả 2 vạch hồng tía ở vạch kiểm tra và vạch kiểm chứng, chứng tỏ mẫu dương tính với virut rota. Khi chỉ có một vạch hồng tía ở vị trí đối chứng, kết luận mẫu âm tính với virut rota.

Que thử được tạo ra theo quy trình này có khả năng phát hiện nhanh (3-5 phút) virut rota trong mẫu phân bệnh phẩm. Độ nhạy và độ đặc hiệu đạt 100%, cao hơn so với các kit thương mại hiện nay. Ngưỡng phát hiện là 1,6 x 104 hạt virut/ml. Do sử dụng nguồn kháng thể tự sản xuất nên có thể chủ động được nguồn sinh phẩm, đồng thời giá thành thấp hơn các kit nhập ngoại đang lưu hành trên thị trường, nên có thể dễ dàng áp dụng trong thực tế.

Với những ưu điểm đó, nghiên cứu này mới đây đã được Cục Sở hữu trí tuệ cấp bằng độc quyền giải pháp hữu ích số 2-0002106 công bố ngày 25/9/2019. Đây là một điều vô cùng kịp thời, khi mà theo Cục Y tế dự phòng (Bộ Y tế), mùa Đông – Xuân, với điều kiện thời tiết ẩm ướt, nhất là sự gia tăng trong giao lưu đi lại vào dịp Tết Nguyên đán và lễ hội mùa xuân sẽ là điều kiện thuận lợi cho các bệnh truyền nhiễm lây lan, bùng phát, đặc biệt là bệnh tiêu chảy do virut rota gây ra.

A. PCT APPLICATION

Filing deadline

The deadline for a PCT application to enter Vietnam National Phase is 31 months from the earliest priority date (or the international filing date if the international application does not claim priority right).
A grace period for nationalizing a PCT application in Vietnam is not obtainable unless the late filing is caused by force majeure (wars, natural disasters, accidents…).
Required information

Full name(s), address(es) and nationality(ies) of the applicant(s);
Full name(s), address(es) and nationality(ies) of the inventor(s);
Country(ies), application number(s) and filing date(s) of the priority application(s);
PCT application/publication number.
Required documents

An English version of the specification (preferably in Word format).
Power of Attorney (POA) (notarization/legalization is not required).
Notification of the Recording of a Change (PCT/IB/306), Amendments under Article 19, Amendments under Article 34, if any.
Note: Priority document(s) is/are NOT required

B. PARIS CONVENTION APPLICATION

Filing deadline

The deadline for filing an application claiming priority under the Paris convention is 12 months from the priority date.
There is no grace period for claiming priority right, unless the late filing is caused by force majeure (wars, natural disasters, accidents…).
Required information

Full name(s), address(es) and nationality(ies) of the applicant(s);
Full name(s), address(es) and nationality(ies) of the inventor(s);
Country(ies), application number(s) and filing date(s) of the priority application(s);
International Patent Classification (IPC) of the invention. If this information is not provided, fees for classifying the invention will be charged.

– The deadline for submitting the priority document(s) is 3 months from the filing date. This deadline is non-extendable.
C. REGULAR APPLICATION

Required information

International Patent Classification (IPC) if the invention.
Required documents

An English version of the specification (preferably in Word format)

Patent/utility solution applications will be formally examined, then published, and then substantively examined.

Formality examination
– For non-PCT application, formality examination period is 01 month from either (i) the filing date (if all required documents have been submitted), or (ii) the date on which the NOIP receives all required documents, whichever is later.

– The time to start formally examining a national phase application of a PCT application is the first day of the 32nd month from the earliest priority date. Formality examination period is 01 month from either (i) the start date (if all required documents have been submitted), or (ii) the date on which the NOIP receives all required documents, whichever is later.

– In practice, this period is often delayed due to the NOIP’s backlog. If the examination results are positive, the NOIP will issue a Decision on acceptance of the application. On the contrary, the NOIP will issue a formality Office action.

Publication
A valid patent/utility solution application is published in the 19th month from either (i) the earliest priority date or the filing date (if no priority is claimed), or (ii) within 02 months from the date of official acceptance of valid application, whichever is later.

Substantive examination
– For patent applications, a request for substantive examination together with examination fees must be filed within 42 months from the earliest priority date;

– For utility solution applications, a request for substantive examination together with examination fees must be filed within 36 months from the earliest priority date;

– It is impossible to extend the deadline unless the delay is caused by force majeure (wars, natural disasters, accidents…).

– Under the regulations, the substantive examination period is 18 months from either (i) the publication date (if the request for substantive examination is filed before the publication date), or (ii) the request date (if the request is filed after the publication date). However, this period is often delayed in reality.

– Vietnamese examiners often rely on the examination outcomes of large patent offices, such as the USPTO, EPO, JPO, KIPO, SIPO, etc., of the equivalent applications when examining Vietnamese applications. Thus, to accelerate the examination process, you are advised to inform us of equivalent patents once they are granted.

Annuities

– The validity term of an Invention Patent is 20 years and of a Utility Solution Patent is 10 years from the regular filing date;

– Annuity is required to be paid only after a Patent is granted;

– The first annuity is paid together with granting fees. Next annuities are due on the anniversary of the granting date;

– A grace period of 6 months is available with a surcharge of 10% of the fees for each month late.
