"It was unavoidable." That's not always true — and it shouldn't be the end of the conversation. 🚩

Bed sores have specific, documented causes: failure to reposition, inadequate nutrition, inadequate hydration, and failure to keep skin clean and dry. Every nursing home resident at risk is supposed to be assessed and protected against all four.

The question isn't whether a bed sore was possible to prevent — it's whether the facility actually did what they were required to do. Too often, the documentation shows they didn't.

Q&A: The nursing home administrator says I'm not allowed to take pictures of my mom's injuries. Is that right?
No — a nursing home cannot legally stop you from photographing your loved one's visible injuries, bruises, or other signs of neglect. That's your right as a family member.

A couple of tips: if a wound is covered by a bandage, don't remove it yourself to get the photo — wait until it's naturally redressed by staff or a doctor. And be mindful of privacy — don't capture other residents or staff in the shot.

"They have dementia, they didn't know what they were doing." 🚩
That might be true for the resident — but it's not an excuse for the facility. If a resident has a documented history of aggression, it's right there in their chart. The nursing home's job is to assess that risk and put supervision in place to prevent it from hurting someone else.

A dementia diagnosis doesn't let the facility off the hook. If they knew about the risk and didn't act, that's a failure to protect your loved one — and they can be held responsible for it.

"We don't know how that happened." 🚩

If you've heard that from a nursing home about a bruise or scratch on your loved one, it deserves a second look. Unexplained injuries are one of the most common signs of resident-on-resident abuse — and a resident with dementia often can't tell you what happened.

The facility's job is to know what's going on in that building and to have the supervision in place to prevent it. Document everything, take photos, and ask for the incident report. If you're getting the runaround, don't wait — contact the state hotline or a nursing home lawyer in your area.

Check This Number Before Choosing a Nursing Home

Every nursing home's antipsychotic medication rate is public — it's right there on Medicare's Nursing Home Compare website. It shows what percentage of residents are on these medications who don't have a diagnosis that requires them.

A high rate can be a red flag. It may mean a facility is reaching for medication instead of investing in real, individualized care.

Before you place a loved one in a nursing home — or if they're already in one — pull up that number and come prepared with questions.

Did You Give Consent for That Medication?

Did anyone call you before your mother was started on an antipsychotic? If not, that's not just an oversight — it may be a violation of federal law.
Nursing homes are required to get informed consent from you or the responsible party before starting medications like Seroquil, Haldol, or Abilify. If your loved one was started on one of these and nobody reached out, ask questions now.
Before you sign off on any behavioral medication, ask two things: what non-drug approaches were tried first, and what are the specific risks for someone her age with her conditions. You're entitled to real answers before you agree to anything.
