Most employees don’t realize the moment documentation begins is often the moment the situation has shifted.

What may feel like “routine feedback” can actually be the start of a paper trail. That paper trail is often used to justify future discipline or termination decisions.

This doesn’t always mean the outcome is already decided, but it does mean you should become more aware, more precise, and more intentional in how you communicate and respond.

If something is inaccurate, incomplete, or taken out of context, address it professionally. Small details matter when they are written down.
