A case can look ordinary until one document changes the way everything connects.
It may be a safety policy, a repair request, a training record, a prior complaint, or a contract showing who was responsible for what.
That one record may explain what should have happened, who had authority, and why the condition was not corrected.
The case does not always change because of something dramatic. Sometimes it changes because the right document finally reveals the structure behind the incident.
