Most Pressure Ulcers Are Avoidable

Most pressure ulcers—also known as bedsores—are preventable with consistent, attentive care.
Basic prevention includes regular repositioning, routine skin checks, and proper hydration and nutrition. When this care is ignored, skin breakdown becomes inevitable.

Families should always perform skin checks during visits. Pay close attention to any area that is in constant contact with a surface, like the tailbone area, the buttocks, and the backs of the heels. The first stage of a bedsore is a red mark that does not turn white when pressed.

If a wound appears, families can ask:
“What is the turning schedule?”
“When was this first documented?”
“What treatment plan is in place?”

If you suspect neglect of any kind, don’t delay in raising early concerns and strongly advocate for your loved one’s safety. Bedsores can progress rapidly, leading to sepsis, bone infections, and even death.

California AB 2135 focuses on improving transfer and discharge safeguards for residents in care facilities. The bill is still in the early stages—but the issue it addresses is critical for the elderly and the vulnerable.
Rushed or poorly coordinated discharges can put vulnerable residents at risk, especially when medical needs aren’t properly communicated or proactively planned.

Families should feel empowered to ask:

• “What is the care plan at the next location?”
• “Who is coordinating the transition?”

A Push for Safer Staffing in Nursing Homes

The “Nurses Belong in Nursing Homes Act” (S. 3886) has been introduced at the federal level, focusing on minimum nursing presence and staffing expectations.

Why does this matter? Because staffing levels are closely tied to resident safety. When facilities are understaffed, basic care such as turning, monitoring, and basic hygiene are often sacrificed, resulting in elder neglect.

This bill is still in the early stages, but it reflects a growing recognition that safe staffing is not optional—it’s foundational. All facilities must provide enough trained nurses to meet the needs of their patients.
California already has minimum staffing requirements, intended to protect patients in Skilled Nursing Facilities. Unfortunately, sometimes the bare minimum isn’t enough. If you see signs of understaffing in a loved one’s care setting, trust your instincts and start asking questions early.

Wandering & Elopement: A Preventable Safety Failure

For individuals living with dementia, wandering is a predictable behavior associated with the disease. Intervention isn’t optional, but a necessary and crucial part of their basic care.

Well-run facilities offer individualized supervision plans, provide secured and monitored exits, and communicate clearly during shift changes.

Families with loved ones who are at risk for wandering and elopement should ask facilities simple but important questions:
“How do you track residents with wandering behaviors?”
“What happens during staff transitions?”
“How are exits monitored?”
“Are adequate protections also in place at night?”

Vague or inconsistent answers can be a red flag for supervision breakdowns. Early awareness maximizes safety—and helps prevent avoidable harm.

California SB 434: Why RCFE Eviction Protections Still Matter

California SB 434 aimed to strengthen protections against improper “lock-out” or refusal-of-entry situations in Residential Care Facilities for the Elderly (RCFEs). The bill did not pass—but the issue it addressed remains very real.

When a resident is denied re-entry or pushed out without adequate planning, the harm is often immediate—from disruption in care to medication lapses to transfer trauma. For medically fragile seniors, even a short gap in care can have serious consequences. Although SB 434 failed, families should stay alert to discharge and access issues—and ask clear questions about facility policies before problems arise.

Why the Older Americans Act Still Matters for Elder Safety

The Older Americans Act Reauthorization Act (S. 2120) is still making its way through the legislative process—but the programs it supports are already essential to elder safety.
Services funded under this Act—like caregiver support, meal programs, and community-based assistance—help reduce isolation and ensure that seniors have consistent oversight and care. The act also funds the Long-Term Care Ombudsman’s Office, which investigates and advocates for seniors in long-term care facilities like nursing homes and memory care units. When those systems are strong, gaps in care will shrink—and fewer gaps mean fewer opportunities for neglect.
