Most families don’t get to shop around. You get a phone call from a discharge planner and a day or two to decide. That’s just how it works.

So here’s what I actually look at. Not the lobby. Not the star rating by itself.

I want the inspection history. I want staffing numbers, not staffing promises. I want to know who really owns the place, because the name on the sign isn’t always who’s calling the shots. And I want to read what the surveyors actually wrote down, not the summary.

A good rating doesn’t mean nothing happened. A bad rating doesn’t prove a case by itself. Either way, you have to go look.

I put together a full guide — where to find every one of these records, what a one-star rating actually means, and the 20 largest one-star facilities in Kentucky right now. Link below.

Kentucky nursing home ratings and inspection reports help families evaluate facilities and identify potential concerns before choosing.

$23.6M Verdict Against PruittHealth in Nursing Home Death

A Georgia jury just handed down $23.6 million against PruittHealth. The case: a 76-year-old man named John Owens Jr. — a retired teacher, a coach, a former scout for the Pittsburgh Pirates — died six weeks after the nursing home allegedly gave him morphine, lorazepam, and hydrocodone. None of it was prescribed for him.

Two nurses were covering 46 residents that night.
The family says the drugs weren't for his health. They were to keep him quiet.

This happened in Georgia, but don't think it can't happen here. Kentucky law says the same thing: KRS 216.515 gives nursing home residents the right to be free from chemical restraints, period, unless a doctor puts it in writing for a real medical reason. That's what the law says. It's not always what happens on the night shift.

If someone you love suddenly got "sleepy" or "confused" after a medication change and nobody can tell you why — don't let that go. Ask for the medication administration record. Ask who ordered it. Ask why.

We look into these cases across Kentucky. If something's not adding up, call us at (859) 413-3900 or reach out online. Free, confidential.

Signs of nursing home neglect Kentucky families often miss 👇

1. Left too long in a soiled brief or unchanged bedsheets
2. Hunger, not getting regular meals
3. Dhydration is more common than most families realize
4. Falls, from a lack of supervision and observation

The pattern isn't usually one big incident. It's basic daily care quietly going undone.

And unless you're visiting often, you may not see it. Families are frequently kept in the dark about what's happening with their loved one and most don't know what they don't know until the records get reviewed.

Nursing home neglect in Kentucky doesn’t usually look like abuse. It looks like small things nobody wants to make a fuss about.

A bedsore that “just appeared.” A second fall in a month. Weight loss on a resident who needs help eating. A parent who’s suddenly sedated. Call lights ringing with nobody coming.
