Has your child celebrated their 18th Birthday? If so, congratulations on having an adult child according to the law. If you still want to be informed about their medical conditions and procedures you need to have your son or daughter sign a form giving you permission to receive medical information and make medical decisions for them if they are unable to do so. Yes you may have this for your senior aged parents but without one for you adult aged children you maybe left in dark and unable to make decisions for your adult child if they are unable to do so.
I have attached a link to a Florida Bar provided form called a Designation of Health Care Surrogate.

Get this form sign by your adult child with two witnesses present.
You will be glad you did!
