Need a passport? In person service available, $130 for ages 16 and older $100 for under 16 plus an acceptance fee of $35 for each person applying.

What’s needed:
Form DS-11 (https://eforms.state.gov/Forms/ds11.pdf)
Original Birth Certificate
Photo ID
Passport photos (Walgreens)

For kids under 16 both parents must be present or a Statement of Consent form will need to be provided as well as a front and back copy of the ID of the parent not in attendance and this form needs to be notarized (UPS stores can do this if don’t know a notary)
