Some problems a court order can't fix, and there are some issues that need to be resolved between the parents rather than in court. Some people need to file a child custody suit, a child custody modification, or a suit against their co-parent, and there is a non-legal, much less expensive way to deal with their problem.

If you leave things out of your divorce decree or child custody order, it doesn't exist! Now what do you do?

You will likely have to open a SAPCR - Suit Affecting the Parent-Child Relationship. That will lead to a final court order affecting the children. For all else, you might have to file a second lawsuit to prove your ownership.
