For students who wish to avail of notarization services, please make sure you bring any identification that verifies you are already 18 years old or above. If you are still a minor, you need to bring your parent(s) with you.

Dear Students,

For notarization, the Affiant—the person making the statement—must personally appear before the notary public. In the case of a Parental Consent document, the Affiant is your parent, not you. Only your parent must appear for notarization, as they are the one giving consent. You, as the student, are not the Affiant and should not be the one having the document notarized.
