( Additional Requirements for Late Registration of (BC) )
- Supporting documents with CORRECT NAME, DATE OF BIRTH, and PLACE OF BIRTH. Any of the Following:
1. Baptismal Certificate
2. Voter Registration Record
3. Form 137 / Transcript of Records ( readable dry seal )
4. NCMF / OMA Certificate ( for Muslim applicant )
* Acceptable Picture ID's
1. Driver's License
2. Digitized SSS ID
3. GSIS E-Card / UMID Card
4. Voter's ID
5. Digitized BIR ID
6. PRC ID / OWWA ID / IBP ID
7. Old College ID / Alumni ID
8. Old Employment IDs ( with SSS E-1 form )
* Supporting Documents ( atleast 3 )
1. Seaman's Book
2. NBI Clearance
3. Police Clearance
4. Old School ID's
5. Form 137 or Transcript of Records ( TOR ) with readable dry seal
6. BC PSA of child with signature of mother at informant
7. Yearbook with solo picture
8. Old Documents issued at least 2 years ago that show picture and signature
9. Land Title

( brown passport ) or issued prior to May 1995
1. Old Passport and photocopy of passport pages 1,2,3 ( amendment )
2. PSA Birthcertificate to determine complete middle name
3. Proper ID's with complete middle name
( Green Passport )
1. Original Passport and photocopy of passport first page and last page.
2. VALID ID ( original and photocopy )
( Machine Readable Passport - MRP ( Maroon Passport )
1. Original Passport and photocopy of passport pages 1,2,3 ( amendment ) & last page.
