HSE statistics suggest that most Slips and trips incidents occur when floor surfaces are contaminated with water, talc, or grease, etc.

Slips: occur when the foot and floor cannot make effective contact, usually when something has been spilled, or when the shoe sole and floor finish are incompatible.

Trips: occur when an obstruction prevents normal movement of the foot resulting in loss of balance, usually caused by objects on the floor or uneven surfaces.

Here is a very important topic related to Mobile Equipment which is the pre use inspection, The inspection is very important to make sure your equipment is safe to use and it should be conducted at the beginning of each shift. I know some of you may think this is a waste of time but believe me by experience I found the conducting of inspection and identifying some areas of concerns will help you by more that 60% to prevent an incident. The inspection should be completed by operator as he is the best person who know the Equipment Here some areas that you need to inspect:

Pre-use checks

1-Tyre pressures – ensuring that pneumatic tyres, if fitted, are inflated to the proper pressure
2-Tyres checks – signs of damage such as swarf, nails or other embedded material, cuts and bubbles
3-Parking brake, service brakes, and steering gear to ensure that they are working efficiently. Fuel, water and oil levels (internal combustion engines) to check for leaks
4-Batteries of battery-operated lift trucks to check:
-that they are adequately charged and leak free
-that the charger is switched off
-the charge lead disconnected and properly stored
-the battery retention device is in place.
5-Systems for lifting, tilting and manipulation, including attachments are working properly
6-Hydraulic systems should be free from obvious leaks, and hydraulic fluid levels should be correct when the fork arms are in the parked position
7-Audible warning signals, lights, and mirrors, if fitted, should all be in good working order.
8-Horn and reverse alarm which should be working ok.

You need to know the different between routine and non-routine work to you can decide if you need safety document (permit to work ) risk assessment and method statement or not.

Examples:

Walk through around your working area (inspection)-routine job.

Drive your vehicle from camp to your working area-routine job.

Open panel and see the connection cables- non-routine job.

(High risk work) Working at height-confined space-hot work-lifting-LOTO- all is non-routine job.

If your work is routine then you no need to have risk assessment and method statement to do the job but if your work is non-routine then you need to have all related document
