A coroner has concluded that neglect contributed to the tragic death of baby Zachariah Millin at Queen's Hospital.

Punam Sood represented his parents, Sinita and David, at the Inquest and will now work with the hospital in relation to a negligence claim.

The Trust has apologised to the family and admitted the standard of care at Queen's Hospital did not meet an acceptable standard.

The coroner acknowledged that the Trust has introduced several measures to improve maternity care in response to Zachariah's death, including increased staffing levels at triage and training in pain management and monitoring and extended pre-eclampsia testing.

Mark Bowman secured settlement for Tyson who suffered catastrophic injuries after clinicians failed to diagnose and treat a serious bacterial infection.

Tyson was just three years old when he became unwell with symptoms including a high fever, headache, vomiting and seizures. Despite showing signs of a severe infection, Tyson was sent home from A&E without appropriate investigation, monitoring or treatment.

Tyson’s condition deteriorated significantly. He went on to develop sepsis and was eventually admitted to an Intensive Care Unit, where he required a induced coma.

The infection caused widespread damage to Tyson’s body, including kidney failure and brain injury, requiring intensive care. He also underwent emergency surgery on both legs due to the infection, but his right leg could not be saved and was amputated below the knee.

The Trust has admitted failures in his assessment, diagnosis and treatment, including delays in administering antibiotics that contributed to his injuries.

The focus is now on the claim is securing the lifelong care and support Tyson will need, from prosthetics and rehabilitation to appropriate education.

Jamie Green secured settlement for Natalie* after medical staff failed to get appropriate consent from her for breast reconstruction surgery.

Natalie underwent breast implant surgery which later became infected, requiring further surgery to remove the implant.

Natalie was not told, however, about the risk that the implant may need to be removed within six months post-operatively or the use of surgical mesh. When the implant was removed, the mesh could not be taken out and was left in place.

Over the following months and years, Natalie was left with an open wound as the mesh surfaced through her skin, requiring over 20 procedures to gradually cut it away.

Following expert evidence confirming negligence, Jamie successfully secured a settlement, providing Natalie with financial security and access to the treatment she needs.
