Comprehensive Procedure for the Issuance of ISO 9001:2015 Certification to the Client
1. Introduction
The process for issuing ISO 9001:2015 certification is comprehensive and is designed to ensure that the client organization has implemented a robust Quality Management System (QMS) that conforms to the requirements of the international standard. This document outlines, in exhaustive detail, the stages involved in the certification process, the responsibilities of the certification body, and the obligations of the client throughout the entire lifecycle of certification—from application to final issuance.
2. Application and Contract Agreement
2.1 The client initiates the process by submitting a formal application for ISO 9001:2015 certification to the accredited certification body.
2.2 The certification body provides a detailed application form to gather relevant information, including:
Legal name and address of the organization,
Scope of activities,
Locations (sites),
Total number of employees,
Shift patterns, and
Exclusions, if any, from the standard.
2.3 Based on the application, the certification body reviews the feasibility and competence requirements for the audit.
2.4 A formal certification agreement is signed between both parties, detailing terms and conditions, confidentiality clauses, dispute resolution procedures, and payment terms.
3. Preliminary Review (Optional Stage)
3.1 Upon request or necessity, the certification body may conduct a gap analysis or preliminary assessment to review the client’s readiness for certification.
3.2 This stage helps identify any major nonconformities or weaknesses in the QMS before proceeding to formal audits.
4. Stage 1 Audit – Documentation Review and Readiness Assessment
4.1 The certification body appoints a competent lead auditor and audit team as per ISO/IEC 17021-1 guidelines.
4.2 The Stage 1 audit is conducted either remotely or onsite, focusing primarily on:
Review of QMS documentation (Quality Manual, procedures, policies),
Evaluation of compliance with ISO 9001:2015 clause requirements,
Understanding of the client’s processes and interactions,
Readiness for Stage 2 audit.
4.3 A report is generated highlighting:
Areas of concern,
Observations,
Potential nonconformities, and
Recommendations for readiness.
5. Stage 2 Audit – Certification Assessment
5.1 The Stage 2 audit is an on-site comprehensive assessment to evaluate the effective implementation of the Quality Management System.
5.2 Key focus areas include:
Review of operational controls,
Management review records,
Internal audit records,
Customer satisfaction data,
Control of nonconformities and corrective actions,
Performance against quality objectives.
5.3 The audit findings are categorized as:
Major nonconformities,
Minor nonconformities,
Observations.
5.4 In case of major nonconformities, the client is required to submit a Corrective Action Plan (CAP) within a stipulated time (usually 30 to 60 days), followed by a follow-up audit if required.
5.5 Minor nonconformities and observations are to be addressed and closed internally by the client with documented evidence submitted to the certification body.
6. Certification Decision
6.1 Upon successful completion of the Stage 2 audit and closure of all nonconformities:
The audit team submits the audit report and recommendation to the Certification Decision Committee (CDC), which is independent of the audit team.
6.2 The CDC reviews all documents, including:
Audit reports,
Corrective actions,
Auditor competence and impartiality,
Any risk factors associated with the client organization.
6.3 Based on this review, the CDC decides whether to:
Grant certification,
Delay certification pending additional information or corrective actions, or
Deny certification.
7. Issuance of ISO 9001:2015 Certificate
7.1 If the certification decision is favorable, the certification body proceeds to issue the ISO 9001:2015 Certificate of Conformity.
7.2 The certificate includes:
Name and address of the certified organization,
Scope of certification,
Date of initial certification and validity period (usually 3 years),
Unique certificate number,
Accreditation mark (if the certification body is accredited),
Logo of the certification body.
7.3 A soft copy is sent electronically for immediate use, followed by a hard copy certificate via post or courier.
7.4 The certified organization is granted the right to use the certification logo in accordance with the Logo Usage Policy, which explicitly prohibits the use of the certification logo on product packaging, in a manner that suggests product certification.
8. Post-Certification Surveillance
8.1 Surveillance audits are conducted at least once a year to ensure continued compliance with ISO 9001:2015 requirements.
8.2 These audits verify:
Effectiveness of implemented processes,
Continuous improvement efforts,
Internal audits and management reviews,
Handling of customer complaints and feedback.
9. Recertification
9.1 At the end of the 3-year certification cycle, the client must undergo a recertification audit, which is similar in scope to the Stage 2 audit.
9.2 If successful, a new certificate is issued with revised dates, and the cycle repeats.
10. Certificate Suspension, Withdrawal or Cancellation
10.1 The certification body reserves the right to suspend or withdraw the ISO certificate if the client:
Fails to address nonconformities,
Violates terms of the certification agreement,
Misuses the certification mark, or
Stops maintaining an effective QMS.
10.2 Written notices and adequate time for corrective actions are provided before such decisions are enforced.
11. Confidentiality and Impartiality
11.1 All client data, audit findings, and certification decisions are maintained confidentially.
11.2 The certification body ensures impartiality through independent reviews, conflict-of-interest declarations, and periodic evaluations.
12. Public Listing and Verification
12.1 The name of the certified client is listed on the certification body’s public registry or website, including certificate number and validity.
12.2 Verification of the certificate's authenticity can be requested by third parties.
